John “Jack” Ustar, 62, of Apache Junction, AZ passed away on April 19, 2023 following a short battle with aggressive cancer.

Jack was born on April 5, 1961 in Cleveland, Ohio, youngest son of the late Elaine and Maynard Ustar. Jack grew up in Willoughby, Ohio and graduated from Lake Catholic High School where he was an accomplished gymnast.

Jack found his paradise on earth when he moved to his adopted home of Phoenix, Arizona in 2000. Being the genuine people person that he was, Jack embarked on a sales career after high school where he found his true calling. Jack loved his customers as much as they loved him and earned the nick name “Five-Star Ustar."

Jack was preceded in death by his parents; grandparents Wanda and Walter Wojtkiewicz; Helen and John Ustar. Jack is survived by his brother and lifelong best friend Jim Ustar; nieces Kay (Brandon), Sara Ustar (Michael), nephew Jimmy Ustar, his beloved partner Greg, many treasured aunts, uncles, cousins, close friends, co-workers and countless other loved ones.

Whether you knew him as Jack, John or Johnny, everyone felt his incredibly kind heart and giving nature. A Celebration of Life service will be held in Ohio and announced at a later date.

Jack’s passing came as a shock to our family and donations in lieu of flowers would be greatly appreciated. The funds will be used to cover the cost of cremation, burial and any other costs associated with his celebration of life memorial service.
