Jeremy S. Smith, 45, passed away peacefully on Tuesday, November 30th, with his wife Trevor Kathleen Smith by his bedside.
Jeremy was born to Roy Smith and Brenda Smith (McLean) on March 29, 1976 in Columbus, Ohio. Jeremy, his parents, and his older brother Geoffrey Smith moved to Crystal Lake, Illinois his sophmore year of high school.
