Jack W. Hazlewood was born on September 26, 1943, and passed away peacefully on December 28, 2025, at the age of 82.
Jack proudly served his country in the United States Army from 1964 to 1967, attaining the rank of Sergeant (E-5). He served with honor in the 82nd Airborne Division, 3rd Battalion, 325th Infantry Brigade, stationed at Fort Bragg, North Carolina.
