The Care Team Hospice - Crown Point

The Care Team is a hospice care provider in Crown Point and the surrounding communities. The Care Team's hospice team includes physicians, nurses, social workers, volunteers, and chaplains who provide patient-centered care during the final phases of a patient’s disease process. Care is provided where those in need call home, so they can be surrounded by what they treasure most. Our hospice team is available 24 hours a day, 7 days a week, to ensure patient and family needs are met.
