Mindy was born on May 9, 1959 to Dorothy and [Father's name] in [city of birth]. After graduating from Seaside High School in [high school graduation year], [he/she] went on to work as one of Seaside's favorite bank tellers at Astoria bank and Lewis & Clark Bank.
In [year they met], she met Larry Cozart, and they wed in [marriage year], going on to have two children, Kurtis and Melissa "Missy".
Mindy was preceded in death by her fathers, [first name] Wilson and Harry Lusby. She is survived by her husband, Larry, her mother Dorothy, her two children Kurtis and Missy, her 6 grandchildren, and her two sisters.
She loved her children and grandchildren more than anything in the world. She could often be found spending her free time...
