Jeffrey was born the son of Jeffrey and Staci (Johnson) Chase on Sunday, October 1, 1989, in Potsdam, NY. Jeff grew up in the area graduating from Newark High School class of 2007. Jeff's whole world revolved around his daughter, Addison. He was an activist at heart, supporting causes like BLM and woman's rights. He enjoyed being in nature, listening to music, and gardening.

Jeff will be remembered by his fiancee, Brooke Toth; children Addison Bulman, Erik and Noah Smith; his parents Jeff (Staci) Chase; grandparents Marlo (Connie) Chase, Howard (Barb) Johnson and Pam (Roland) Collins; brothers, Matthew Chase and Mackenzie (Hailey) Chase; and many uncles, aunts and cousins.
