Tammy Short — I've been quality assurance nurse with this company for 2 years. The work culture cannot be beat. The CEO is extremely kind, caring, intelligent and well respected among all staff.

Emily Hite — My mom was in a nursing home in Indy, hospice was also assisting mom with meds and care. They discharged her in October of 2018, because she had gained some weight. However, November 1, she had lost 15 lbs and Paradigm Hospice was called back in. They did assist with her care , then the first of December, she took a huge downhill slide. Hospice was called immediately, they started a "comfort kit" on mom. Mom passed away on December 6, 2018. My sister was with her. Thank you to the nurses who started the comfort kit. Mom passed peacefully, she was 90. They had all the equipment she needed, and assisted with her care. Thank you, being 400 miles away, its hard being so far away, but I know between Forum at the Crossing nursing staff and Paradigm, she was in great hands

Diane Kostyshyn — Poor coordination among staff and constant turnover of RN/Case Manager - From the start, my Mom was bombarded with phone calls from various staff members to come instead of a coordinated approach. She is very bright and has her mind at 88 but with all the calls and constant changes in personnel, she started to become very confused and the care actually added to the stress rather than helping. Having a single point of contact to coordinate would have helped in the beginning. I tried to give feedback and it slightly improved. To their credit, each person tried to be helpful but they were tripping over each other. It was very difficult to get a schedule for the nurse and the frequent turnover caused us to keep updating and informing on my father's rare condition, adding stress again to the family. The final straw was when the wrong equipment was ordered (Hoyer lift instead of sit to stand) because "the equipment company didn't have the right lift". Again sending the wrong equipment, not training the assisted living staff. I would not recommend using this agency. Growing pains as they are expanding but overall poor care for my father (and mother who is the caregiver). The one postive was Alecia, the aid who gave my father his baths. He really liked her and she is to be commended for her excellent care.
